REPORT OF ALLEGED VIOLATION OF UNDERGROUND
​UTILITY SAFETY AND DAMAGE PREVENTION ACT

Important information.  Please review carefully: 

​Per N.C.G.S. § 87-120(e), North Carolina 811, Inc., as the Notification Center, receives reports of alleged violation of the Underground Utility Safety and Damage Prevention Act.  

The report form provided here was developed by the North Carolina Underground Damage Prevention Review Board.

North Carolina 811, Inc. is not involved in the enforcement or adjudication of reports of alleged violations of the Underground Utility Safety and Damage Prevention Act.   

North Carolina 811, Inc. forwards all reports of alleged violations, along with any accompanying information, to the North Carolina Underground Damage Prevention Review Board.  

​The North Carolina Underground Damage Prevention Review Board and the North Carolina Utilities Commission are responsible for enforcement of the Underground Utility Safety and Damage Prevention Act. 

Per the requirements of N.C.G.S. § 87-120(e), within 10 days of receipt of a report of alleged violation, North Carolina 811, Inc. will contact any person who is the subject of a report to inform them of the alleged violation.  Likewise, North Carolina 811, Inc. will provide to the North Carolina Underground Damage Prevention Review Board copies of any response and accompanying materials from any person who is the subject of the report of an alleged violation.  Please provide complete contact information for the person who is the subject of the report so that North Carolina 811, Inc. can notify them about the alleged violation.  If you do not provide complete contact information for the person(s) who are the subject of your report,  North Carolina 811, Inc. may write to you to request the address in order for North Carolina 811, Inc. to be able to contact such person(s) about your report of alleged violation. 

​North Carolina 811, Inc. does not monitor reports of alleged violations or response to reports of alleged violations, nor will North Carolina 811, Inc. be able to provide you information about the status of your report of an alleged violation of the Underground Utility Safety and Damage Prevention Act.
